Painful bladder syndrome
What is painful bladder syndrome?
Painful bladder syndrome, also known as interstitial cystitis, is a chronic disease characterized by pelvic pain associated with urinary disorders, in the absence of urinary tract infection.
This complex pathology profoundly affects quality of life, with persistent pain, frequent urination and constant discomfort.
What are the symptoms?
- Pelvic pain or burning, often related to bladder filling,
- Urgent and frequent need to urinate, even at night,
- Sensation of pressure or discomfort in the lower abdomen,
- Pain during intercourse,
- No urinary tract infection despite symptoms,
- Variability of symptoms over time.
What are the causes?
The precise causes remain poorly understood, but several hypotheses have been put forward:
1️⃣ Alterations to the bladder wall,
2️⃣ Chronic inflammation of the mucosa,
3️⃣ Pelvic nervous system disorders
4️⃣ Immune or autoimmune factors,
5️⃣ Trauma or repeated irritation of the bladder.
When should you consult Dr. Laurence Peyrat?
It is recommended to seek medical advice as soon as :
- Persistent unexplained pelvic pain,
- Disabling urinary disorders (emergencies, frequency),
- Disabling symptoms affecting your daily life,
- Failure to treat other urinary causes.
Dr. Laurence Peyrat offers a comprehensive assessment to make an accurate diagnosis and guide treatment.
How is the diagnosis carried out?
Diagnosis is based on :
1️⃣ Detailed discussion of symptoms and their impact,
2️⃣ Exclusion of other urinary causes (infections, stones, tumors),
3️⃣ Complementary examinations (urinalysis, cystoscopy under anaesthetic),
4️⃣ Urodynamic tests to assess bladder function.
What are the treatment options?
1️⃣ Medical treatments
- Anti-inflammatory drugs, analgesics or drugs that modify the bladder mucosa,
- Neuromodulatory therapies to relieve nerve pain,
- Intra-vesical injections (e.g. heparin instillations, Botox).
2️⃣ Rehabilitation and comprehensive care
- Pelvic re-education to reduce muscular tension,
- Stress management and psychological support,
- Lifestyle adaptation (diet, hydration).
3️⃣ Surgical options
- Rarely necessary, but possible in the case of severe resistant forms,
- Procedure to increase bladder capacity.
