To best adapt treatment, it is essential to understand the exact cause of bladder weakness through targeted investigations.
What are the objectives of the explorations?
- Identify the type of incontinence (stress, overactive bladder, mixed, etc.),
- Assess bladder and urethra function,
- Determine the severity of the disorder,
- Search for possible associated causes (infections, anatomical anomalies),
- Choosing the most appropriate treatment for each patient.
What tests can be performed?
1️⃣ Consultation and clinical examination
- Precise recording of symptoms, their duration and impact,
- Examination of the perineum and assessment of the pelvic floor muscles,
- Simple tests such as the cough maneuver to check for leaks.
2️⃣ Urinalysis
- Urinalysis to rule out infection,
- Measurement of post-micturition residual (amount of urine remaining in the bladder after micturition).
3️⃣ Voiding diary
- Recording of urine output and leakage episodes over several days,
- Gives a better understanding of urinary habits.
4️⃣ Pelvic ultrasound
- Visualization of the bladder, kidneys and pelvic organs,
- Check for urinary tract abnormalities or retention.
5️⃣ Uretrocystoscopy
- Endoscopic examination of the bladder and urethra,
- Used when lesions, tumors or anatomical anomalies are suspected.
6️⃣ Urodynamic investigations
- In-depth functional study of the bladder and urethra,
- Measurement of bladder pressures and volumes,
- Used to detect involuntary contractions or urethral closure defects,
- Indicated especially in complex cases or prior to surgery.
How does urodynamic exploration work?
- Performed in consultation, under supervision,
- Placement of small catheters in the bladder and rectum,
- Progressive filling of the bladder with measurement of pressures and sensations,
- Effort-induced urine leakage test,
- Duration varies, usually 30 to 60 minutes.
