When should a cystostomy be considered?

Cystostomy is a surgical procedure involving the creation of a temporary or permanent opening in the bladder to ensure drainage outside the natural route. This technique is used in many situations where normal evacuation of urine is impossible or contraindicated.

Cystostomy is indicated in particular in cases of :

Severe urethral obstruction (tumors, strictures, trauma) preventing normal urine flow,

✔ Significant bladder dysfunction with inability to urinate spontaneously,

✔ Recurrent urinary tract infections requiring prolonged drainage,

✔ Preparation for or following complex pelvic or urological surgery ,

✔ Palliative management in some patients to relieve symptoms.

Objectives of cystostomy

– Ensure effective, safe bladder drainage,
– Prevent or treat complications associated with urinary retention,
– Improve patient comfort and quality of life,
– Facilitate management of complex situations.


Surgical technique

Cystostomy can be performed using different methods, depending on the clinical context:

Percutaneous cystostomy (under radiological or ultrasound control): minimally invasive method, often used in emergency or outpatient care.
Surgical cystostomy: incision in the lower abdomen to expose the bladder and place a drain (catheter or catheter).

The procedure is performed under local, locoregional or general anesthesia, depending on the situation.
A catheter is placed through the abdominal wall into the bladder to ensure continuous drainage.


Post-operative care

– Check that the drain and skin site are working properly,
– Moderate risk of local infection (can be prevented by antibiotics),
– Management of the catheter: regular changes and local care,
– Possibility of returning home with appropriate equipment and medical follow-up.


Results and follow-up

– Cystostomy bladder drainage effectively prevents distension and renal complications,
– Whether the device is maintained or removed depends on the evolution of the underlying pathology,
